Plague is not eradicated actually: the WHO took a census of 2118 cases in 2003, among which 182 were fatal.

Details:

The most affected countries are Madagascar and the democratic Republic of Congo, where 2005 cases and 177 deceases have been reported.
Having said that, there hasn’t been a true epidemic for a long time.
